Current Iodine Deficiency Disorder Situation
Population
Population: 3.96 million
Population growth rate: 0.76%
Population < 15 years: 19.8%
Birth rate: 12.76 per 1000
Life expectancy at birth: 72.02 years
Infant mortality: 23.5 per 1000
Prevalence of Goiter/ UIE
Median urinary iodine: 102 mcg/L Most recent data report a median urinary iodine of 102 mcg/L.
A 1999 survey of 5,523 schoolchildren, age 7-14, reported a goiter prevalence of 27% (range 12.9% in west Herzegovina Canton to 51.2% in Bosnia Podrinje Canton); median urinary iodine showed 78 mcg/L (range 1-208 mcg/L).
Historically much goiter, but by 1974 a 20 year study showed a significant decrease in the prevalence of IDD since the introduction of iodized salt in the population. In past, as part of Yugoslavia , endemic goiter was located throughout all mountainous areas, with the highest risk found along river banks in deep valleys. IDD not a problem in Vojvodina (Northeast) and along Adriatic coast.
Iodized salt Coverage
Household iodized salt use: 100%. Stable coverage with iodized salt
Salt Situation Analysis Production
Salt in Bosnia and Herzegovina comes mainly from a plant in Tuzla .
Consumption
Use is compulsory for humans, animals, food industry, and coverage is 100%.
Iodine Procurement and Utilization
previously iodized at 5-15 mg KI/kg salt, and from Pag at 10-20 mg KI/kg; levels recently increased to 20-30 ppm. Universal Salt Iodization Program
Information, Education, Communication (IEC) Activities
a national committee and monitoring scheme in place.
Legislation
Law in 2001 stipulates compulsory use of salt iodized at 20-30 ppm KI. Iodized salt prescribed for use by humans, animals, and the food industry.
Program Monitoring and Evaluation
National Coordination Committee for IDD Control includes Ministry of Health, thyroidologists, representatives from public health, legislation, the health inspectorate, nutritionists, and salt producers. It oversees monitoring of salt and nutrition. Status assessed by urinary iodine.
Other Interventions None
Key Lessons Learned
Challenges and Constraints
Future Plans for Sustained IDD Elimination A new survey planned for 2003 to monitor the effects of the increased KI concentration in salt
